LATE RESULTS OF CORRECTIVE MAMMOPLASTICS IN WOMEN AFTER COMPLICATED INTRAMAMMAR POLYACRYLAMID GEL INJECTION

The aim of the work was to study complications, which have been developed in the late mammoplasty period in women with geleomas formation after intramammar injections of polyacrylamid gel (PAAG). Additional aim of work was to estimate different breasts imaging methods efficiency performed for PAAG compartments identification.

Materials and methods. For period from 1999 till 2008 117 women with complications after breast correction by PAAG injection were examined and operated. General clinician methods, instrumental and laboratory methods, ultrasound, mammography and MRI imaging, surgical methods of treatment – gleam excision, mammoplasty, and statistical methods were used.

Results and discussion. The surgery aimed for excision of gel and geleomas and reconstruction of the breast was done with silicone implants. We observed 62 women with complications after retromammar injection of polyacrylamide gel (PAAG). In 1.5–2.5 years from the prosthesis implantation the complication results have been analyzed in 10 patients. It was shown that minimal complications risk was observed in patients with subpectoral and subfascial inframammar implants location. The retorglandular space deterioration level caused direct impact upon the complications risks precipitation. Hence, PAAG debrices located in retromammar space tissues, including pectoral muscle have great significance for the surgical treatment tactics choice.

Conclusions. Repeated surgical interventions performed with the aim of gel and geleomas excision along with septic complications in PAAG location zone comprises the complications risk. Subpectoral as well as inframamar subfascial implants location are followed by complications minimal risk in comparison with retromammar one. MRI was most efficacious method of PAAG location visualization and permitted to determine subfascial compartments of gel, which have been created in the course of wrong technology of PAAG administration.
