THE COMBINED ANTIBACTERIAL THERAPY WITH CEPHTHAZIDIM AND AMIKACINUM IN COMPLEX TREATMENT OF THE SERIOUS INTRA-ABDOMINAL INFECTION

The aim – to estimate empirical combined antibacterial therapy (АBТ) efficiency in serious intra-abdominal infections (IAI) with
cephalosporin of ІІІ generations Cephtum and aminoglycoside of III generation Amicil (Arterium corporations, Ukraine).
Materials and methods. Results of treatment of 85 patients at the age from 18 till 77 years with the serious IAI treated in surgical infections unit during the period with 2005 for 2009 are estimated. All patients have been operated in urgent order after preliminary preoperative preparation. In the postoperative period infusions of detoxic, antibacterial, anti-inflammatory, immune therapy were prescribed
to the patient. As empirical antibacterial therapy cephthazidim on 1 g 3 times a day and amikacinum on 1–1.5 g 1 time a day in a combination with Metronidazol were prescribed.
Results and discussion. The positive clinical effect was noted in 67 (78.8 %) patients (recover – in 36 and improvement – in 31); a
therapy inefficiency – in 18 (21.2 %) patients (absence of effect – in 11 and recurrence – in 7). In off-hospital IAI cases the АBТ efficiency was essentially significant – in 93.3 % rather than at hospital IAI – in 70.9 % (р < 0.05). At the same time in last group of patients
it is necessary to recognize АBТ result significant enough, considering, that the prognosticated lethality at an investigated contingent of
patients exceeds 50 %.
Conclusions. The carried out researches prove high clinical efficiency of a combination cephthazidim and amikacinum as empirical
therapy of a serious IAI (including, caused by P. аeruginosa), that allows to recommend it as the choice scheme in treatment of the given
patients category.
